Oral phentermine Hydrochloride (HCL) is sold for weight loss. It is in the Sympathomimetic family of appetite suppressants (used for the short-term management of exogenous obesity.)

Patient's Current Health Conditions:- 72 Year Old Female- Controlled type II Diabetes- Controlled Hypertension- Contolled Anemia

Received First FDA approval in the United States in 1959, as a short term appetite suppressant for obesity. No new efficacy trials have been performed with the exception of its use as Phen-Fen, its combination with fenfluramine.

Local Anesthetic Recommendation:The local anesthetic of choice for this patient would be Prilocaine 4% free of epinephrine. Epinephrine would be contraindicated due to the restlessness being experienced by the patient as a side effect of the Phentermin. If there is even the slightest suspicion of tachycardia it is best to keep the patient from products with epinephrine as it could lead to a cardiac complication. Our patient currently weighs 155lbs and therefore the math is as follows for the delivery of this LA.4% solution = 4g/100ml = 4,000mg/100ml = 40mg/mlIf the cartridge of Prilocaine contains 1.8 ml, this translates to:40mg/ml x 1.8ml= 72mg per cartridge.The maximum recommended dose on a patient who is between 150-160lbs is 2.7mg/lb. or an absolute maximum dose of 400mg.This means that if each cartridge contains 72mg and the MRD is 400mg:400mg/72mg = 5.5When it comes to LA we do not round up. Therefore, this math concludes that our patient can safely receive up to 5 cartridges of Prilocaine 4% epinephrine free in one day.
