Components of a Wyoming IEP
There are 2 purposes of an IEP:1. To establish measurable annual goals for a studentand2. To state the special education and related services, supplementary aids and services the agencies will provide to, or on behalf of a child.
After the Student's information is filled out at the top of the IEP form next the STRENGHTS, EDUCATIONAL CONCERNS and PREFERENCES Area is filed out by the team* Students strengths are listed*Students interests are listed*Educational Concerns of the studentHow well is the child doing in school?How does the childs disability affect how they are doing in class?
-Here the team describes the academic, developmental and functional needs of the child.-Describe how the child's disability affects the student's involvement and progress in the general education classroom-Describe achievement and performance across services across special education, general education and any interventions that have been tried.
Consideration of Special FactorsHere questions are answered on the child: Does the child have communication needs?Does the student's behavior affect his learning or the learning of others?Is the student blind or visually imparied?Does the student require assistive technology devices?* If any item is answered YES, then it must be addressed on the student's IEP
Extended School YearESY services must be provided to student if necessary the child to receive FAPE considering the child's rate of progress and the effect an interruption in programming will have on that rate of progressor the degree of regression
Transition Services-If the IEP is to be in effect when child is 16 yrs old a box is check marked.-There is an area for putting students vocational education desires, Continuing education, independent living and communicty participation.-Attach age appropriate transition assesments if needed.Post secondary goals and training are listed if applicable-Career and Employment Goals
The proposed courses of study are listed to help the student meet their measurable postsecondary goalsTransfer of Rights at Age of MajorityThe student must be informed that rights under IDEA transfer to the student at the age of 18.Graduation or Program Completion area is filled out if applicable.
Measurable Annual Goals*Statement of measurable annual goals including academic and functional goals that are designed to meet the needs of the studentEach goal must have a baseline, target and method of measurement Benchmarks or short term objectives are required only for students who will take alternate state or district assessments.Periodic reports of progress toward meeting goals needs to be documented.
SPECIAL EDUCATION SERVICESA statement of special education, related services, supplementary aids and services to be provided to student, or on behave of the studentIt is recorded on the form what service the child is receiving, frequency, duration, location and projected start date of service.
RELATED SERVICESThis is where services such as Audiology, Counseling, Occupational Therapy are recorded that the child will receive.The Frequency, Duration, Location and Projected Date of Start are completed on IEP form.Supplementary Aids and ServicesHere is where accommodations, assistive technology and aids are listed that the student will recieve. There are services that are provided to avoid removing the child out of their regular education classrooms
Program Modifications are noted on the IEP form that enable a student to advance appropriately towards their goals.LEAST RESTRICTIVE ENVIRON.Yes/No Questions are answered in regards to the student being removed from the regular classroom only if the nature of their disability is such that the child can not stay in their with the use of supplementary aids. STATE & DISTRICT WIDE ASSESSMENTSIt will be determined how student participates in assessments for the state and district.
