Vapor cloud ignighted and caught fire on BP plant:Distilation chamber was opperated outside of safety regulations.There was no flare to burn off excess vapor or gas.

Levee failure from Hurricane Katrina:The levees that were put in place to protect the city failed and allowed water to come in and destroy the city.
The levees should have been made to withstand a more powerful hurricane.The levees should have been built taller, wider and the metal poles holding them up should have been put in the ground deeper.
The water coming over the levee created a ditch at the base of the levee and as a result the levee failed and fell down.
There should have been a safety flare to burn off the excess vapor.Upkeep sould have been performed.Safety guidelines should have been followed better.
