Cystic Fibrosis

CYSTIC FIBROSIS

What is CF?Cystic Fibrosis is an inherited disease where the muscus glands affect many of the body systems. It affects the cells that produce mucus, sweat and digestive juices. These secreted fluids are normally thin and slippery. But in cystic fibrosis, a defective gene causes the secretions to become thick and sticky. Instead of acting as a lubricant, the secretions plug up tubes, ducts and passageways, especially in the lungs and pancreas.

ACTUAL PROBLEMS

Respiratory System:Abnormally thick mucus plugs the small airways. This contributes to obstruction and inflammation leading to chronic infection, tissue damage, and respiratory failure

Integumentary System:sweat gland produce a large amount of chloride leading to a salty taste of the skin

Respiratory1. Barrel chest 2. Clubbing of the nail beds3. Adventitious lungs sounds4. Difficulty clearing secretions5. Oxygen saturation low6. Recurrent Respiratory Infection7. Nasal Polyps

Clinical Manifestations

GI1. Meconium ileus2. Retention of fecal matter causing ABD distention, RLQ pain3. Intussusception4. Rectal Prolapse5. Failure to Thrive6. Development of Diabetes

Gastrointestinal System:Pancreas, gallbladder, intestinal glands and bile ducts are obstructed by viscous mucus which leads to a malabsorption of fats, proteins and carbohydrates.

Care Plan #1A: tachypnea, increased mucusD: Ineffective airway clearance r/t increased secretions AEB increased work of breathingP: Maintain Patent AirwayI: Postural Drainage & Chest PhysiotherapyE: Respiratory Rate wiithin Normal Parameters

Care Plan #2A: despite healthy appetite, child is underweight for ageD: Altered Nutrition; less than body requirement r/t obstructed digestive glands AEB inadequate weight gainP: formulate dietary planI: Administer high calorie, high protein dietE: Child will gain 1lb per week for 2 months

Care Plan #3A: Salty taste of the skinD: Electrolyte imblance r/t alteration of sweat glad secretion AEB sweat chloride test results >60 mEq/LP: maintain hydration & electrolyte balanceI: administer fluids, monitor I/O, assess skin turgorE: Urine Output 1-2 ml/kg/hr
